Testimonial Permission

I voluntarily agree to provide a testimonial regarding my experience with Cypressene Fitness and give permission for my words to be used in promotional and marketing materials. I understand that:

My testimonial may be used on the website, social media, printed materials, or digital campaigns.
It may be edited for length or clarity but not for content or meaning.
I may request to review the final version prior to public use, if desired.

Photo and Video Release

I grant Cypressene Fitness, its representatives, and employees the right to take photographs and/or video recordings of me during sessions, events, or classes. I authorize the use of these materials for lawful promotional purposes, including but not limited to:

Website, social media, and digital advertising
Print materials or newsletters
Educational or internal training materials

I understand:

I will not receive compensation for the use of my image or testimonial.
These materials will not be sold or transferred to third parties without consent.
